When the default file associations change (e.g. via the File Associations
preference page), the Package Explorer is left showing the icons for the old
associations.

I just fixed this (ancient) bug in for the Navigator (see bug 2175), but the
Package Explorer still has the problem.  The PE apparently does not use
WorkbenchLabelProvider or it would have gotten the fix for free.

Should check the browser views too.
