There is some magic keystroke that I have been hitting a lot in the past few
weeks which puts me in japanese input mode.  I have no idea what keystroke I
have been hitting, and once I get into japanese mode I have no idea how to get
out, other than to exit the program.  

There should be a method to a) prevent this from happening if the user doesn't
want it (an option to turn it off for instance), and b) allow the user to
recover from this if it does happen (a notification that tells the user what's
going on and how to turn it off).

LeftAlt+LeftShift and then release, perhaps?  There is an option, and it is
provided by Windows XP.  See Bug 78081.
