When the launch view reveives a CREATE event for a debug target or thread, it 
performs an "insert" with the new element. This will add the element to the 
end of the parents children which may be the incorrect order (there is no 
sorter in the view, but the debug model controls sorting of debug elements). 
We should instead perform a refresh on the newly created element.
