build I20040318 + new icons from head

The Save and Print toolbar buttons are almost completely invisible when disabled.

The disabled state of the Back and Forward arrows also look poor.

The Up icon in the Navigator and Package Explorer is disproportionately small
compared to the left and right arrows.

The fat arrows take some getting used to.  I think the old ones were more
elegant, and less distracting.
