use_subject_as_username option patch

As discussed on the mailing list this is a feature request to add an option to use the full certificate subject as the username in the broker.

Attached is a patch which adds this feature as option 'use_subject_as_username'
I'm not sure if this is already sufficient and would like some feedback on it. At minimum it can serve as a starting point for future work.

Thanks very much, I've checked the patch and it looks good, aside from some relatively minor points (check return values!). I've committed it and included some fixes/documentation.
