I've noticed that sometimes (not always), after I hit Ctrl+E then close a file using the context menu, the name of the file disappears but the entry remains in the list with an empty label.

Please have a look at the attachment.

I believe the entry should be removed altogether.

Note that sometimes, closing a file will automatically close the Quick Switch Editor window. Obviously, this bug can be seen only in those cases when the window remains on-screen after closing the file.
