The slideout for the commit action on the new git status page should collect commit message + amend option. That's why I would need a checkbox in the slideout. I remember we were chatting about that, but I can't find a bug.

It is very ugly, I am working on styles in bug 372053.  Wanted to release this first to get you going, in case I don't finish that today.

You want a "boolean" parameter like this:
new mCommands.CommandParameter('amend', 'boolean', 'Amend:', false)]);
That last value should be whatever you want the default value to be.
