When you create a new element from the toolbar, 2 commands are executed, so you need to do 2 undo to get the initial state.
Steps to reproduce, using the EMF-Facet example Open Ecore Tabular Editor : 
	1/ Create an Ecore Model
	2/ Select the root package and select in the popup menu, the action Open Ecore Tabular Editor
	3/ Create an EClass using the action Create New Element in the toolbar
	4/ Do 1 Undo : the EClass is removed from the table
	5/ Do 1 Undo : the EClass is removed from the Ecore Model
