I had Orion server running. Some operations were running when my server was restarted. As the result, all running operations became never-ending operations and there was no way to delete them.

The suggestion is to mark running operations as FAILED on server restart.

Done. I also added removing tasks older than month on server start. I think month is just right to safe assume that nobody really cares for them any more.
