In the Navigator if you select something then use the task bar menu "More" and navigate to "Copy To..." or "Move To..." it might result in "More" just being a grey box.  

Actually you can just click on "More" to highlight it and then move the mouse off of it, click somewhere else,  and it's a grey box until you click again.

that's bad.
I just added some focus styling for accessibility, and it looks fine in the item level but as you note, not good in the toolbar.  Will see if there's a quick and dirty fix for M1.

Fixed with 8167462244d3b31f4cdebf3b2bb5e5866ff6b480

Removed the background color from the CSS for the focused case.
The tradeoff is that the focus emphasis on the "More" button is more subtle in the item level (text goes from gray to black), but we had that problem in other cases.  See bug 341522 for complete discussion.
