The search result item contains time stamp in the indexer.
For a recently modified file we should check if the fileMetadat.localtimeStamp  equals to that time stamp. If not we should do a quick check if the file really contains the search string.
We should grey out such files so that user understand those files are stale in the indexer.
This will be  specially helpful right after replace all action in the search page.
