When parsing the XHTML contents for headers to put in the table of contents we may as well record illustrations that have the "title" attribute set and optionally create a list of illustrations to include in the publication.

For tables the "caption" element is the one to look for. The two lists are slightly different in the resulting OPF code and a bit more in implementation, but still so similar that I elect to put them together in one patch.
