Reviewer participant cannot always add a new Review Item element.  For instance in formal reviews PLANNING phase, reviewers cannot add manual review items.  Likewise, in formal reviews PREPARATION phase they cannot add review items from text selections only.  It should be allowed at all times

THe fix is to roemove restrictions so now anybody can add a review item is all phases except when the review is completed
