When I generate the report, I looked at the structure R4EReview.  For the FORMAL review, it seems ok

BASIC review: missing the review type and the end date when the review is completed

INFORMAL review: end date missing when the review is completed

The end date info was not checked in properly.  This is now OK.  For the Basic review, this is the default value and that will be fixed on the report side
