While using formal reviews, a reviewer with the single role of "Reviewer" is not allowed to create anomalies.
A work around is to assign it another role e.g. "Lead"

I tested this and it works fine.  Do you have more details on how to reproduce?

Right now this problem is not reproduceable.  We'll leave it open and add more info if we manage to reproduce it.
