Since GMF 2.4 I'm missing the note tool in the palette, next to the enlarge and minimize buttons. :(

Tested on ecore diagram editor and our product on top of indigo, the note tool is displayed.

This is another plug-in which removes this tool. For instance papyrus did it, see bug 341897.

Try to start from a fresh modeling package, and install one by one feature to know which one is removing the tool. If you installed papyrus, try first without it.

You may also use one of the plug-in eclipse views, to find who contributes to org.eclipse.gmf.runtime.diagram.ui.paletteProviders extension point.
