Currently, R4E prevents the addition of new Review Items once it reaches the decision phase.
However if a follow up is needed, new Review Items might be needed to be associated to this same review. in order to continue with the follow-up, 

It is then suggested to remove the checks so new Review Items and anomalies can be created.

The checks for formal review phase was removed.  It is now possible to add review items and anomalies anytime except when the review is marked as reviewed or is completed

Not able to add anomalies e.g. tested in REWORK phase, 
creation of anomalies shall be supported as linked anomaies, from Navigator views, from compare editor, single editor, outline, etc.

Correction, the above was tested using the wrong test bed. With latest, the missing items are:

* Not able to create Linked anomalies for Deltas or Selections
* Attempting to create an anomaly from the Outline e.g. a method. Gives the dialogue for a comment rather than an anomaly.

First issue is now fixed
Second issue is intended behavior when we try to add an anomaly on an element that already has an anomaly.
