I opened a task in my list and was very surprised to find an unsubmitted comment that I thought I had submitted days before. The task did not have an outgoing arrow in the task list and the clear outgoing option in the context menu was grayed out. I guess the submit failed for some reason after the outgoing state of the task was cleared.

Sam, do you have steps to reproduce? The outgoing state is not generally lost when submitting fails but I have seen this happen and wondering what triggers this.

No, I have no idea what caused it. I've never noticed it before.

I think there's a good chance I closed the task editor during the submit. I don't know if that could have caused it.

sorry, no steps to reproduce either.  Not sure if it's related, this happened shortly after we had a very slow repository with socket timeouts.
