Since offset information is long lost by the point the model query extension is asked for information, the best thing to do is update line 49 to use the position independent retrieval method:
    List trackers = new ArrayList(manager.getTaglibTrackers());

Regrettably, we lost the ability to only propose custom tags from tag libraries already declared at the cursor offset in a document.  I was rather fond of it being that smart.
