"The Templates Preferences page is accessed from Window | Preferences | PHP | Templates"

Home > User Guide > Tasks > Using Templates
"A template must be defined in the Templates list in the Template Preferences page, accessed from Windows | Preferences | Templates, before it can be used."

Both should be "Window | Preferences | PHP | Editor | Templates"
