1. Open an Bugzilla task
2. Click on 'Add Attachment'
3. Choose a zip file to attach
4. click next twice

On the Attachement Details page, the 'Advanced' section is empty. It should not apear if there are no advanced options

Which version of Mylyn are you using? Do any controls appear if you resize the dialog?

resizing helped. The controls were hidden. may be because am on 1400 x 1050 resolution.
I will leave it up to you how you wish to resolve the bug :)
