CommandContributionItem.update() does not provider ActionContributionItems mechanism to show both icon and image on a toolbar. 
It should be easy to add a boolean attribute "forceText" to the org.eclipse.ui.menus.menuContribution element. 

This would be very useful because I could use the IMenuService to populate custom toolbars with icon-and-text CommandContributionItems and do not have to switch the whole architecture to the action framework.
