New PHP dialog should default to the file name text box. Currently it defaults to the folder (even if I chose the folder already).

Repro:
1. Create PHP project
2. Create a Folder
3. Now, right click on folder's name, and choose new file

Expected:
Default text box should be the file name

The focus is now the file's name
