This is more obvious if I trying this out in a runtime workbench.

1.  Open a java file
2.  Set a breakpoint
3.  RMC->Disable Breakpoint

The enablement state of the breakpoint is updated in the Breakpoints View immediately.  However, the marker at the ruler still shows that the breakpoint is enabled.  The marker shows the breakpoint as disabled after a little while.

If I try this out in the first workbench.  The enablement state of the ruler marker is updated more quickly.  However, opening a new editor and trying it again made it slower.  There is about a 1-2 seconds delay.
