There are several pages where 'Use defualt' make no sense, e.g. see bug 81069 or
bug 81180.

It should be possible to only have the 'Apply' button. Currently a page can say
'noDefaultAndApplyButton'

IMO 'Apply should always be there, can't think of a reason why you wouldn't want
that.

The only problem with us always putting apply there is that people are 
expecting it not to be there if they set noDefaultOrApply to true.

There are no plans to implement this change.

I don't think this is fixed, I see several preference pages without "Apply", see for example "Execution Environment". Or is this bug only about the option to activate the Apply button independent from the Default button?
