Views need to have tool bars.

Ed, the view toolbars seem to be functioning properly with your latest 
submission.  Are there any known problems?

- If a view doesn't have a drop-down menu, R21 presentation draws a disabled 
triangle while 2.1 drew nothing.
- If the controls don't all fit on the title bar it should draw a horizontal 
line separating the title bar and the controls (toolbar) area

Lines appear to be being drawn between the toolbar and the title bar as well 
as the toolbar and the view contents.

Let's just leave the disabled view menu icon as it is, this does not seem 
worth the effort of removing.  Ed do you agree?
