The terminate action should be on the main Debug menu, along with resume and 
suspend.

This will simplify keyboard accessibility.

Added terminate action to the debug menu.
Also ensured that the menu accelerators were that same as the context menu of 
the debug view.  Added an accelerator for the Disconnect action.
Please verify.
