Pressing the Code-Review+2 button on this change [1] results in an "Internal server error" error message popping up.

After clicking the "Continue" button and refreshing the page, I can see that the approval was actually applied.

If I then press "Submit", I get the error: "The page you requested was not found, or you do not have permission to view this page".  On cancelling that, I get "Error submitting change".  Then, on cancelling that it goes back to the "The page you requested ... " error which repeats.  I can only resolve it by going back to the main changes list, and then I can see that the change was in fact submitted.

I'm also getting the internal server error when adding comments on changes.
