If you open the code formatter page with extra large fonts the Blank lines 
page is cut off

STEPS
1) Set your fonts to Extra large 
2) Open the Code Formatter preference page
3) Select Show
4) Select the Blank Lines tab

We have to limit the dialog's size and can't let it grow so all is visible.
We added a sash between the controls and the preview and also the dialog window
is resizable.

I have just rechecked and if you maximize the dialog it all fits fine.
