I've run into this problems a few times now and always considered it some network issue. But now I do think this is something else.

For CQ8958 I simply cannot create a new piggyback CQ. After selecting the dependency and pressing "next" I get:

Trying later doesn't fix the problem ;-)

Seems to work now. Perhaps this was a point-in-time issue (database?)
