Reproduction (not always but very often):
Select a method in the java editor deep down in your code and press Ctrl + Alt + H. Then the call hierarchy gets computed for the parent callers. Now before using the call hierarchy click somewhere in the editor (in the opened file). Now use the call hierarchy. Most of the time while expanding the tree there are no further callers.

For me the severity is critical because I waste many minutes a day missing that functionality.

Like I wrote in bug 470554, this behavior may be caused by the MoreUnit plugin.
