This is apparently an SWT bug in how it manages its item's images. We should likely move the defect over to the SWT component for them to have a look at... I was able to reproduce this issue using Eclipse Version: 4.3.2 Build id: M20140212-0800. Changing the 'Version' to 4.3.2 SWT folks, this is a recurring error for which the only explanation is that there's a mismatch between the image specified for an icon and the 'cache' of icons maintained by SWT. The only odd thing that I'm doing here is to add the new ToolItem at a specific index rather than at the end (which is the common case). This is a one-off bulk update. (The last one in the triage migration). Created attachment 242690 [details] Screenshot showing duplicate icons Using Eclipse Version: Luna (4.4) Build id: I20140501-0200, perform perspective re-ordering multiple times (say 50). After a while i could see that the perspective image icons are all looking the same. Hovering over the icon gives the correct perspective name. There is no error logged.
