When RFT is installed and launched on a new workspace, if the Debug toolbar is available and visible, then all debug options are enabled. 

The same behavior is observed on base Eclipse 4.2.2 only in the Java perspective. The Debug perspective is able to enable/disable the icons accurately. Eclipse version I saw this issue on is
