The Formal Review icon in the Navigator View is incorrectly showing the Informal Review icon.

I can't replicate but I also observed that both Informal and Formal reviews switched to the Formal Review when opened.

Both icons are correctly installed in the /icons folder and the settings look OK at first glance.

The Basic review behaves correctly.

When creating the Formal Review, Informal icon appears, then close and re-open the Formal review,then the Formal ICON shows in the review navigator
