This seems to be happening after indexer has been run for a while with some files changed after indexing started. I've seen, for example, "192/-74 sources, 742 headers"

I've seen this twice now. Today I saw it when I refreshed my folder after updating (externally) my source control. I have Refresh on Access enabled.
