1. Window > Open Perspective > Other... > Java > OK
2. Window > New Window
3. Window > Open Perspective > Other... > Debug > OK
4. The debug toolbar is now visible.
5. Window > Open Perspective > Other... > Java > OK
6. The debug toolbar is still there.

Confirmed that this is, in fact, caused by Bug 320478.

I have a fix for this that will go in Friday.

In essence we where calling ActionSetManager.hide and show too aggresivley leading to problems with its reference counting.
