I'm using Eclipse 4.2M3 (also tried with last nightly of 2 Dec 2011), and I'm getting the annoying "Item not added" error on at least two scenarios:

What happens:
1. Open Eclipse
2. I have my Java files open of the last time I used it. In the opened Java file I see a blue line, that means that line is focused, but I don't see any cursor. Also, the Project explorer seems to be the current focused element on the GUI.
3. If I click anywhere in the file to put the cursor on it, the "Item not added error appears.

To avoid it, if I click on the file tab (the one with the name of the .java file) and the I click on the file, everything works (I think the editor panel gets the focus after clicking on the tab, because it changes color)

Another way to get the same error:
1. Hover over any member of the Java file to get the Javadoc hover.
2. Click on Javadoc hover.
3. Click on Java code again.

I've attached the exception I get on the Error log view (same for both)

This bug hasn't had any activity in quite some time. Maybe the problem got resolved, was a duplicate of something else, or became less pressing for some reason - or maybe it's still relevant but just hasn't been looked at yet.

If you have further information on the current state of the bug, please add it. The information can be, for example, that the problem still occurs, that you still want the feature, that more information is needed, or that the bug is (for whatever reason) no longer relevant.

If the bug is still relevant, please remove the "stalebug" whiteboard tag.
