Some databases, such as SQL Server, have a clustered index which can be associated with a primary key.  This needs to be supported in the SQL Model.
This can be done by adding a boolean attribute "clustered" to the UniqueConstraint class, which is the parent of the PrimaryKey class.  The default value of the attribute should be true.
