After Rule Set child elements are disabled and restored, it may happen that the are duplicated in the Review Navigator tree.  Closing eclipse and restarting does not fix the problem

This is probably due to a side effect of bug 358468. Not reproduceable anynmore.

I did fix context menu for Rule Area and Rule Violation elements that were not correct when the element is disabled
