I'm not sure where the "template file" is, but while browsing sites such as 
http://git.eclipse.org/c/
or 
http://git.eclipse.org/c/equinox

I noticed a line that says

The contents of the Git repositories are made available under the terms and conditions of the Eclipse.org Software User Agreement.

And "Eclipse.org Software User Agreement" is linked to an invalid (404) location. Clicking on it goes to 
http://www.eclipse.org/legal/notice.php

I suspect what was intended as 
http://www.eclipse.org/legal/notice.html 
but, that too is actually out of date, and redirects the browser to 
http://www.eclipse.org/legal/epl/notice.php
which is (I assume) what should be used when fixed.
