When adding to a form a new field, the user is asked to select a field. In the next dialog the user can enter a name for that field. While typing a list of proposal is shown. To add a new name to the NLS the uer has to  select the entry "New translated text...".
The form has to be filled out and the changes have to be confirmed. The form closes and the previous shown dialog is shown again.

If I am changing (for whatever reason) the name again, the currently added entry will not be shown again in the list of proposals.

removed iplog flag from ticket and added iplog flag to attachment
