New Item Dialog in "advanced: false" has a problem with setting the name input value. If you type name in the Name field and imiediatly hit "Enter" the value returned in the callback function is empty.
If you confirm the dialog with OK button the value is passed. The same if you focus out of the field and then confirm with Enter.
I suppose change of the value is recored only on focusing out of the input field.

I fixed in via hack: before I use the value I focus to  the OK button. Works for FF 4 and IE 8.
