Not easy to reproduce and have to be code reviewed,
as sometimes the "Selections" icon for newly created review items displayed the disabled decorator, 
however this element is not supposed to be disabled / enabled

This could be true for pretty much any element of the review navigator.  It does not happen and is very hard to reproduce

A related problem that also rarely happens is that some elements become disabled when the "Show disabled elements" default filter is toggled on or off.

This is probably due to a side effect of bug 358468.  Not reproduceable anynmore
