In the case of a servlet handling an SSL request that never returns to the select set dispatcher, it is possible for a flush to only partially flush the content to the SSL buffer.  Flush needs to block until even the endpoint buffers are flushed.

fixed by AbstractGenerator checking isBufferingOutput and calling EndPoint.flush directly if it is.
