Actions that are in an action set and then contributed to the menu via 'org.eclipse.ui.menus' extension point do not appear on the 'Customize Perspective' dialog's 'Command Groups Availability' page.

Example: take a look at the 'Java Debug' actions set: it's 'Open from Clipboard' menu contribution is missing. The relevant code is in /org.eclipse.jdt.debug.ui/plugin.xml.
