I installed Releng Tools into this build, and then hit "Apply Now". This works fine in 3.6 RC4, but completely blows up in 4.0. It seems we are caching stale extension objects somewhere.

Our processing of extensions into model is 1) happening at the wrong time in the lifecycle and 2) not responding to dynamic adds or removes.

I think 1.) is handled by Bug 376486 and 2.) is handled by Bug 376254. Closing as duplicate.
