In 2.3.1, for Crosstab reports the header does repeat on each page, even in the DOC output. I believe the output should automatically print a new header on each page if the option to repeat header is chosen.

Do you mean the header doesn't repeat in 2.3.1 or latest version even the option is selected?

Can not reproduce this issue in 2.3.1 release build. Column headers were repeated on each page if you enable the option in property editor: Cross Tab -> Row Area -> Page Break -> Repeat Column Header.
