Build Identifier: 

Consolidated the table of contents (TOC) from multiple to a single file in org.eclipse.wst.server.ui.doc.user.  As a result, retiring many .ditamap and .xml files.

Removed the extra build_toc.xml files in both plugins.

In addition, added parent-child links in H1 topics.
