The Key bindings preference page is inaccessible for the following reasons:

1) No mnemonics
2) The label for the combo is above it - it needs to be beside it for screen 
readers
3) It is possible to have neither Default or custom selected. Tab to one or 
the other and use the arrow keys to tab to the other one. The radio button is 
deselected but the other one is not.

The radios cannot be read by a screen reader anyways. You should consider 
using an enable/disable with a checkbox and entering that way - the way you 
show it is not intuitive.

1. fixed: (press ALT to see the mnemonics on windows)
2. fixed: all labels are left of corresponding widgets
3. fixed: dumb bug from brand new code
