There have been several bugs causing duplicate menu items.
See bug 14061 and bug 18274.
Need tests to cover these cases.

Added a test (DuplicateMenuItemTest) for testing the workbench menus but I was 
not able to get to the menus of the popups as I actually have to fill the menu 
and the API on IResourceNavigator is protected. Once we figure out how to do 
this we can add in tests for it too.

All that is left in this bug (if I understand correctly) is to add tests.  Could
either of you comment as to whether this bug is still relevent (it's quite old)?

It was a recurrent problem we had in 2.0.  The coverage that Tod already added 
is probably sufficient.  Can close.
